Ministry Health Survey
Privacy Information
What We Collect
We collect only your numeric survey answers (1–5 or "prefer not to answer"), and optionally your grade level and gender if you choose to provide them. We do not collect your name, email address, phone number, or any information that identifies you personally.
What We Do Not Collect
- Your name or username
- Your email address
- Your phone number
- Your Ignite Youth account ID
- Your exact GPS location
- Advertising identifiers
- Persistent device fingerprints
How Results Are Created
Individual answers are combined with other anonymous responses to calculate aggregate scores for each ministry-health dimension. Leaders see only group averages, never individual answers. Results are not shown until at least 5 students have completed the survey.
Who Can View Results
Only the ministry leader who created the survey can view the aggregate results dashboard. Leaders cannot see individual responses. Ignite Youth platform administrators can view usage statistics but cannot view individual student answers.
How Long Data Is Retained
Aggregate results are kept until the ministry leader deletes the survey campaign. The ministry leader can permanently delete a survey and all associated response data at any time from their dashboard.
How a Leader Can Delete a Survey
Ministry leaders can log in to their Ministry Health Dashboard, open any survey, and choose "Delete Survey." This permanently removes the survey record and all associated anonymous response data.
Children and Youth
Because youth ministries may include students under age 13, this survey is designed to avoid collecting personal information and persistent identifiers. We do not ask for birth date, exact age, school name, home address, or any contact information.
Ministry leaders are responsible for reviewing applicable child-privacy requirements and obtaining any permissions required by their church, organization, denomination, insurer, or local law.
Disclaimer
The Ministry Health Survey is an anonymous ministry-feedback tool designed to help leaders better understand student experiences. It is not a clinical, psychological, or diagnostic assessment.
